Poisoning by household products: there is no correlation between ENT and gastrointestinal damage

There is no correlation between ENT and gastrointestinal damage, or parallelism between esophageal and gastric lesions in case of accidental poisoning of children by household products, said Dr Patrick Nisse, the poison control center University Hospital of Lille, his fellow emergency meeting in Paris for the day Emergency Congress.
Pediatric poisonings are common household products, they account for a quarter of accidental poisoning of children. They occur mostly in children aged 1 to 4 years, and more likely in boys (59%).

The places most frequently associated with this type of poisoning are naturally kitchen, bathroom and toilet, where the products in question are used. This is also when they use that accidents usually occur within three quarters of the time in the presence of parents, which tends to prove that they are negligent or reckless analyzed Dr. Nisse.

The bleach products topped the list of products most frequently involved in poisoning by household products, followed by dinner liquids, detergents. Caustics are meanwhile in sharp decline "through the use of caps and reassuring Bitrex, a bittering product" that discourages the child immediately to bring the product to its mouth. In addition, the concentration of these products fell sharply.

Poisoning by household products are asymptomatic in 60% of cases. "However, the risk remains with caustics and solvents," warned the expert.

Unlike accidental drug poisoning, for which it is recommended to throw the child, proceed rapidly to

Gastric lavage or use of activated carbon

These three therapeutic approaches are strongly discouraged in the management of poisoning household product.

Instead, a liquid diet, associated with consumption of bread, is recommended in order to prevent the product from coming into contact with an even larger area of the body. The use of emergency is not necessary in case of accidental consumption of foaming products (soap, laundry by hand ...) or irritants (Machine wash, rinse, multi-purpose products ...).

Regarding the solvent, the risk is variable, explained Dr. Nisse. If the product is volatile and fluid, the risk is serious and should result in a systematic hospital to supervise the child. Otherwise, the monitoring is done at home. In both situations, a diet without milk or fat is required.

Irritating and corrosive chemicals (stain before washing, descaling agents, softeners, oven cleaners, bleach concentrated ..) are dangerous when the pH is less than 2 or greater than 13. The presentation of the product (powder, liquid, gel), the amount ingested and the type of contact will determine the severity of the poisoning.

Ingestion of caustic must however always lead to hospitalization of the child, in order to achieve an endoscopic assessment. This must be done between the 6th and the 24th hour, when the lesions begin to be visible. "There is no need to rush endoscopy", recommended specialist. In case of severe damage, parenteral or enteral nutrition is recommended.

But in all cases of poisoning by household products, the emergency physician should bear in mind that "there is no correlation between the ENT and gastrointestinal damage, and there is no parallelism between lesions of the esophagus and those of the stomach. "
