Cirrhosis of liver failure
Hepatic encephalopathy is a serious complication of liver cirrhosis and
occurs later. This disease is characterized by neurological signs secondary to
the toxicity of ammonia accumulated in the brain and nervous system.
Hepatic encephalopathy
Hepatic encephalopathy is manifested by
- A asterixis or flapping tremor which are repeated and involuntary
movements of the hands "wing beats"
- A characteristic foul odor of the breath: the liver foetor. This sign is
due to excessive accumulation in the bloodstream of ammonia and other products
that are not destroyed by the liver.
- Other neurological signs involving drowsiness, mood changes, behavioral
and personality disorders of consciousness.
Cirrhosis can also lead to cancer of the liver. Nodules regenerations can
escalate and turn into cancerous tumors.
Biological parameters
Different biological parameters are perturbed during cirrhosis and can
confirm the disease.
Bilirubin: Bilirubin is a substance derived from the destruction of red
blood cells. In case of impaired liver function, bilirubin is accumulated in
the bloodstream.
Prothrombin - Rate Factor V: These are substances produced by the liver and
involved in blood clotting.
These rates are reduced in patients with cirrhosis.
Enzymes in liver cells (transaminases): These are enzymes found in the
liver cells. In liver cirrhosis, the cells are destroyed, thereby releasing
these substances. Enzyme levels are increased.
The degrading enzymes (alkaline phosphatase, gamma-glutamyl transferase)
These enzymes are normally degraded in the liver (via bile). In cirrhosis, they
are accumulated and the rate is increased.
Rate of albumin: Albumin is a protein synthesized by the liver. In
cirrhosis, the levels in the blood is reduced.
Liver biopsy for microscopic examination usually confirms the diagnosis of
cirrhosis.
Abdominal ultrasound shows the structure and appearance of the liver
(granular, nodular, increase or decrease in volume) and complications
(dilatation of the portal vein, ascites, enlargement of the spleen, ...).
Digestive endoscopy may be necessary to search for gastroesophageal varices
for.
